Raffle Giveaway Terms & Conditions

1. Eligibility

• The giveaway is open to individuals who are 18 years or older.
• Participants must be legal residents of the United States or Canada.
• Employees, affiliates, and immediate family members of Functional Medicine CE/PharmToTable LLC are not eligible to participate.

2. Entry Rules

• No purchase is necessary to enter.
• Entries must be submitted by 03/28/2025 at 11:59 PM EST.
• Participants may enter by filling out the form on FunctionalMedicineCE.com.
• Each participant may submit one entry.

3. Prize Details

• The prize includes a free ticket to our April 4, 2025 Symposium.
• The prize is non-transferable and cannot be exchanged for cash or other alternatives.

4. Winner Selection & Notification

• The winner will be chosen randomly on March 29, 2025 from all eligible entries.
• The winner will be notified via email within 48 hours after the drawing.
• If the winner does not respond within 72 hours, a new winner may be selected.

5. Publicity

• By entering, participants agree to allow Functional Medicine CE to use their name, likeness, and entry for promotional purposes without additional compensation, unless prohibited by law.

6. General Terms

• Functional Medicine CE reserves the right to disqualify any participant who violates the terms or engages in fraudulent activity.
• The giveaway is void where prohibited by law.
• Functional Medicine CE is not responsible for technical failures, lost entries, or any other issues beyond its control.

7. Agreement to Terms

• By entering the giveaway, participants acknowledge they have read and agree to these terms and conditions.
